You may already have a preferred platform, but here are some popular options: Survey Monkey (Free with premium option) Typeform (Free with premium option) Google Forms (Free) Zoho Survey (Free with premium option) JotForm(Free survey options) Survey Anyplace Send your survey through your regular affiliate communication channels. However is not that easy to make affiliates fill those surveys. If you add an incentive like a report or maybe a contest where they can earn an amazon card or something it will help getting more responses.
